Terms and Conditions – Intensive Registration Deposit

Your story is still unfolding, and this chapter is about hope, growth, and renewal. Welcome to Choose Recovery Intensives, where you will be supported, challenged, and equipped to step fully into the next chapter of your healing journey.

Confidentiality

All workshops and sessions at intensives will be conducted in a private, confidential setting. No information will be shared without the explicit consent of the participant unless there is a risk to the client or others.

To secure your spot in an upcoming Intensive at Choose Recovery Services, a $100 deposit is required. Please review the following terms in full before submitting your payment.


1. Deposit Requirement

  • A non-refundable $100 deposit is required to place a hold and reserve your space in the Intensive.

  • This deposit is credited toward the total cost of your Intensive.

  • The deposit may only be used for an upcoming Intensive and is not valid for other services or programs.

2. Payment in Full

  • Full payment for the Intensive is due no later than 30 days prior to the scheduled start date.

  • If a payment plan is arranged in advance with Choose Recovery Services, the deposit will be applied to the total balance and the payment plan terms must be followed.

  • Failure to meet the agreed-upon payment plan deadlines may result in forfeiture of your spot and any payments made.

3. Use and Validity of Deposit

  • The deposit has no cash value outside of its use toward an Intensive.

  • It must be applied within 12 months of the original registration date.

  • Deposits not used within this period will be forfeited unless an extension is granted in writing.

4. Cancellation and Refund Policy

  • Deposits are non-refundable under all circumstances unless the program is canceled by Choose Recovery Services (see Section 5).

  • If you are unable to attend, your deposit may be:

    • Transferred to another upcoming Intensive within the 12-month validity window

    • Transferred to a partner or designated individual with advance written notice and approval

  • No-shows or cancellations made within 14 days of the Intensive will result in forfeiture of all payments made.

5. Program Cancellation by Choose Recovery Services

  • In the rare event that an Intensive must be canceled or rescheduled by Choose Recovery Services (due to low enrollment, emergencies, or other reasons), you will be offered:

    • The option to transfer your credit to a future Intensive


6. Transfer Requests and Deadlines

  • Requests to transfer your deposit or registration to another Intensive or individual must be submitted in writing no later than 14 days before the scheduled start of the program.

  • Requests submitted after this window may not be accepted.

7. No-Show Policy

  • Participants who do not attend the Intensive without prior written notice will be considered no-shows, and all payments, including the deposit, will be forfeited.

8. Assumption of Risk & Limitation of Liability

  • By participating in the Intensive, you acknowledge and accept all inherent risks. Choose Recovery Services is not liable for personal injury, loss, or damage to personal property during the Intensive.

  • Participants are encouraged to consider their personal readiness and consult with appropriate healthcare providers as needed before attending.

10. Acknowledgment and Agreement

By submitting your deposit, you acknowledge that you have read, understood, and agree to abide by these Terms and Conditions. You accept responsibility for meeting all payment deadlines and communication requirements outlined above.


This is a DEPOSIT for the Intensive, which will be in-person, in Spanish Fork UT
and does not include accommodations for the four days.
