Terms and Conditions

1. Program Overview

Our intensives are designed to support individuals or couples in focusing on core issues, building emotional resilience, and strengthening relational dynamics. These in-person, multi-day experiences offer focused support and guided skill-building with experienced providers. Intensives are led by our experienced team of trained professionals, including coaches and therapists.

2. Intensive Schedule and Location

Location: Spanish Fork, Utah
Dates: August 4-7, 2026
Time:

Tuesday: 5:00pm - 8:00pm
Wednesday – Friday: 9:00 am – 5:00 pm

Please note:

One meal is provided each day of the intensive, along with snacks and beverages throughout the day

3. Registration

Once you register, if you are new to our program, you will be contacted to schedule a meet and greet with the assigned intensive host.
Upon registration, you will receive information about what to expect, a packing list, and an invitation to join our group chat to connect with other participants.

Participants agree to:

Complete any required documentation and assessments (if applicable).
Communicate openly with facilitators.
Engage respectfully and responsibly during all sessions.

4. Fees and Payment

Total Fee: $1950 (includes intake/meet and greet session).

Payment in full is required at the time of registration. Payment plans are available upon checkout. Select from Afterpay or Klarna to set up a payment plan that works for you.

5. Cancellation and Rescheduling Policy

Due to the planning and time invested in preparing for each intensive, our cancellation policy is as follows:

No refunds will be issued.
If you are unable to attend, your full payment will be credited toward a future intensive or other qualifying services.
If the intensive is canceled by Choose Recovery Services, it will be rescheduled, or a full credit will be issued. Credits must be used within 12 months of the original intensive date.
Participants should notify us as early as possible to reschedule.
Please note that no-shows are treated as cancellations without refund. If a participant does not show, a credit will not be issued.

6. Confidentiality

Sessions are conducted in a confidential, private setting. All facilitators and staff are bound by confidentiality agreements.
Participants agree to maintain the confidentiality of all shared personal information.

Your personal information and participation details will not be shared without your written consent, except in the following circumstances:

Immediate risk of harm to yourself or others.
Legal obligation to report (e.g., abuse or neglect).

7. Participation and Code of Conduct

Participants are expected to:

Arrive on time and attend all scheduled sessions.
Participate with openness and respect.
Avoid the use of alcohol or non-prescribed substances before or during the intensive.
Maintain the confidentiality of other participants.

Recording sessions or recording other participants is not permitted without explicit consent.

Choose Recovery Services reserves the right to dismiss any participant whose behavior is disruptive or harmful, without refund.

8. Emergency and Medical Information

Participants are required to provide:

An emergency contact.
Disclosure of any relevant medical or psychological conditions that may impact participation.

Participants are encouraged to bring necessary medications and insurance information.

Please notify us of any allergies, medications, or medical conditions in advance so we can make necessary accommodations.

We will do our best to accommodate allergies. A kitchen is available for food storage and reheating if you prefer or need to bring your own food.

9. Assumption of Risk and Liability Waiver

By registering, you acknowledge:

You are voluntarily participating in this intensive.
You understand that emotional discomfort or distress may arise during the process.
Choose Recovery Services and its providers are not liable for any emotional, physical, or relational outcomes resulting from participation.
This intensive does not replace individual therapy, medical treatment, or legal advice.
You assume full responsibility for your personal well-being during the intensive.

Acknowledgment and Assumption of Risk

I, the undersigned, acknowledge and voluntarily elect to participate in the above-described event offered by Choose Recovery Services. I understand that events of this nature may include a variety of physical, outdoor, and experiential activities, which may include but are not limited to cold plunge or other temperature-based therapies, paddle boarding, canoeing or other water activities, zip line, ropes courses, yoga, stretching, breathwork, sauna, hot tub, hiking, nature walks, and group process work such as psychodrama, bioenergetics, or other therapeutic exercises.

I understand and acknowledge that participation in these or similar activities involves inherent physical, emotional, and environmental risks, including but not limited to slips, trips, falls, strains, sprains, cuts, bruises, allergic reactions, exposure to extreme temperatures, emotional discomfort, unpredictable weather, and interactions with natural elements such as water, wildlife, or insects.

I affirm that I have been advised to consult a physician beforehand, that I am physically and mentally capable of participating, and that I voluntarily assume all risks of injury, illness, or complications arising from my participation. All such activities are voluntary and may be declined without penalty.

Release of Liability

In consideration for being permitted to participate, I, for myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Choose Recovery Services, its owners, directors, employees, contractors, and agents from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death resulting from my participation, whether caused by negligence or otherwise.

Medical Treatment

In the event I require medical care due to injury or illness during or after participation, I authorize Choose Recovery Services and its representatives to obtain such care at my expense. I accept full financial responsibility for any related medical costs.

Voluntary Participation

I affirm that my participation is voluntary and that I may refuse or discontinue any activity at any time without penalty. I confirm that I have disclosed any known physical or psychological conditions that could affect my ability to participate safely.

Indemnification

I agree to indemnify and hold harmless Choose Recovery Services, its owners, directors, employees, contractors, and agents from any and all claims, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising out of or related to my participation.

Governing Law & Severability

This agreement shall be governed by and construed in accordance with the laws of the State of Utah. If any provision of this agreement is found invalid, the remainder shall continue in full force and effect.

10. Informed Consent

Participation in this intensive implies informed consent and agreement to:

Actively participate within your emotional and physical capacity.
Be aware of and respect your own boundaries and those of others.
Communicate with facilitators about any concerns or triggers.

Facilitators will maintain professional ethics, and participants may request breaks at any time.

11. Disclaimer

Participation in an intensive does not guarantee resolution of the issues being addressed. Outcomes vary depending on individual circumstances, readiness, and ongoing effort outside of the intensive setting. Ongoing therapeutic support may be recommended afterward. Choose Recovery Services can provide referrals or continuity of care.

12. Consent and Acknowledgment

By registering for this intensive, you confirm that:

You have read and understood these Terms and Conditions.
You consent to the policies and expectations outlined.
You accept full responsibility for your participation.
