Appropriateness for Participation

Choose Recovery Services strives to ensure that each participant is placed in the program that will best support their needs.

Choose Recovery Services reserves the right, at its sole discretion, to determine before or during the retreat that participation is not appropriate. This determination may be made if a participant’s needs exceed the scope of the program, a higher level of care is recommended, participation would pose a safety concern to the participant or others, or continued participation would significantly interfere with the experience of the participant or the group.

If Choose Recovery Services determines that a retreat is not an appropriate fit, we may recommend alternative services or referrals. Any applicable credit or refund will be handled in accordance with our cancellation and payment policies or at the sole discretion of Choose Recovery Services.

Registration reserves your space pending completion of any required intake process. Participation is contingent upon Choose Recovery Services determining that the retreat is an appropriate fit based on the information provided during the introductory call.

Confidentiality

All sessions and group activities will be conducted in a private, supportive environment.
 No personal information will be shared without explicit consent, except in situations where there is risk of harm to self or others.

Fees and Payment

Total Cost: $6,499 - $8,299
Includes: Lodging, nourishing meals, all scheduled activities, and retreat materials.

Cancellation & Credit Policy

Due to the significant preparation and resources devoted to each retreat:

• No refunds will be issued for cancellations.
  
  

• If you are unable to attend, notify us with at least 14 days notice and the full amount paid will be credited toward a future intensive or retreat with Choose Recovery Services to be used within one year.
  
  

• If you are unable to attend, please notify us as soon as possible so we can help you select another event.
  
  

• If the retreat is canceled by Choose Recovery Services due to unforeseen circumstances, it will be rescheduled.
  
  

• "No-shows or cancellations made within 14 days of the Intensive will result in forfeiture of all payments made."

While our retreats are designed to create a supportive environment for healing and growth, no specific outcomes can be guaranteed. Each participant’s experience and results will vary.

Liability Waiver:

Acknowledgment and Assumption of Risk

I, the undersigned, acknowledge and voluntarily elect to participate in the above-described event offered by Choose   Recovery Services. I understand that events of this nature may include a variety of physical, outdoor, and experiential activities, which may include but are not limited to cold plunge or other temperature-based therapies, paddle boarding, canoeing, or other water activities, zip line, ropes courses, yoga, stretching, breathwork, sauna, hot tub, hiking, nature walks, group process work such as psychodrama, bioenergetics, or other therapeutic exercises. I understand and acknowledge that participation in these or similar activities involves inherent physical, emotional, and environmental risks,including but not limited to slips, trips, falls, strains, sprains, cuts, bruises, allergic reactions, exposure to extreme temperatures, emotional discomfort, unpredictable weather, and interactions with natural elements such as water, wildlife, or insects. I affirm that I have been advised to consult a physician beforehand, that I am physically and mentally capable of participating, and that I voluntarily assume all risks of injury, illness, or complications arising from my participation.

Release of Liability

In consideration for being permitted to participate, I, for myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Choose Recovery Services, its owners, directors, employees, contractors, and agents from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death resulting from my participation, whether caused by negligence or otherwise.

Medical Treatment

In the event I require medical care due to injury or illness during or after participation, I authorize Choose Recovery Services and its representatives to obtain such care at my expense. I accept full financial responsibility for any related medical costs.

Voluntary Participation

I affirm that my participation is voluntary and that I may refuse or discontinue any activity at any time without penalty. I confirm that I have disclosed any known physical or psychological conditions that could affect my ability to participate safely.

Indemnification

I agree to indemnify and hold harmless Choose Recovery Services, its owners, directors, employees, contractors, and agents from any and all claims, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising out of or related to my participation.

Governing Law & Severability

This agreement shall be governed by and construed in accordance with the laws of the State of Utah, USA. If any provision of this agreement is found invalid, the remainder shall continue in full force and effect.  

I acknowledge and agree that, while attending the retreat in Costa Rica, I am solely responsible for complying with all applicable laws and regulations of Costa Rica. 

Agreement and Understanding

By registering, I acknowledge that I have read and fully understood this General Liability Waiver and Release of Claims, that I am at least 18 years of age, and that I voluntarily agree to its terms.
